Consumer Complaint: I saw a new dentist at XXXX XXXX on XX/XX/ in XXXX Colorado for a simple teeth cleaning - preventative care 100 % covered by my XXXX XXXX insurance. I provided my dental insurance information with XXXX XXXX XXXX Colorado XXXX XXXX XXXX ID XXXX. The office manger there has repeated the correct information over the phone twice or more monthly and promises they will file a claim and not try to collect from me. However, to date, they have not filed any kind of a claim with XXXX XXXX nor provided proof of service and medical records so that I can file the claim myself. Both I and XXXX XXXX have repeatedly reached out together via phone ( through XXXX XXXX 3-way call ) and secure XXXX XXXX email system to get them to file the claim or provide proof of service. Per XXXX XXXX 's contract with XXXX XXXX, they can not send a patient to collections without first submitting a claim. However, they have sent me to collections. ( copy attached ). No matter what we do, neither the collection agency nor the Dentist will stop. They even send nasty text messages that I can not opt-out of. If they can not provide proof of service and medial records they can not send me to collections, so is what they are doing illegal? How can it be stopped? Consumer Complaint: Two years ago, after purchasing my first home, I suspected the presence of mice in my new home. On XX/XX/XXXX, I hired XXXX for a mice inspection. XXXX offered a free inspection at no charge. They did the inspection and suggested a one-time treatment. They also verbally notified me that the fee for the treatment will be a one-time payment of {$100.00}. I offered cash for the service, and they refused to accept cash. Then I accepted to pay the one-time fee of {$100.00} with a credit card. XXXX crew handed me his XXXX for the credit card payment and my signature. After applying the treatment, the crew left, and I have not heard from the company. After 3 months, on XX/XX/XXXX, I noticed recurring transactions on my credit card from XXXX. I noticed that XXXX charged me 3 times already, once every month. I called the XXXX customer service asking for an explanation, and they informed me that I have a contract with XXXX and will be charged $ 102 monthly for a year for treatment, with an option to terminate the contract by paying a {$500.00} termination fee. I informed them that XXXX never informed me about any contract, and I neither agreed to such a contract nor signed any such contract. In response, XXXX emailed me a copy of a contract ( Attached ). Upon reviewing the contract, I suspect when the XXXX crew told me to sign for the one-time credit card payment, that was the contract and not merely a signature for payment. I feel they tricked me into signing something I did not agree to. I discussed my suspicion with XXXX over a phone call and requested to terminate my contract without a termination fee as I was not made aware of a contract, and they refused. I called my bank and informed them about this fraudulent activity. My bank refunded me for the two additional months when I was charged wrongfully and placed a hold on future payments to XXXX. I called XXXX on multiple occasions to resolve this issue, and they refused help. In XX/XX/XXXX, XXXX sent this contract to a collection company. I take very good care of my debts and have an excellent credit score. I pay all my bills and mortgage on time every month. I'm seeking your advice and assistance on resolving this matter.
