Consumer Complaint: I have been trying to resolve this debt collection for about a year now. Finally someone picked up the phone today and I let them know that I wanted to settle my account but they said they had to transfer me. As soon as they transferred me, the next person gave me a number to call that is disconnected. There are many things I would like to do that I cant because of this being on my credit but they arent giving me a way to settle it or make a payment. I tried calling back the original number and now no one will answer.

There are dates that the billing department representative billed the wrong insurance company : XX/XX/XXXX to XX/XX/XXXX : XXXX XXXX XXXX XXXX XXXX XXXX XXXX, PA XXXX ( insurance coverage - primary ) Billed XXXX XXXX on XX/XX/XXXX for following amounts ( {$400.00}, {$370.00}, {$370.00}, and {$200.00} Additional coverage was with XXXX as well during this time but I had a Federal plan from XX/XX/XXXX to XX/XX/XXXX ( insurance coverage-secondary ) Billed XXXX XXXX XXXX on XX/XX/XXXX XXXX XXXX {$64.00} XX/XX/XXXX XXXX XXXX and XXXX {$180.00} My copay was XX/XX/XXXX for {$28.00}, XX/XX/XXXX {$55.00}, and XX/XX/XXXX {$83.00}. XXXX XXXX XXXX was my secondary and they paid for my copay which is correct on XX/XX/XXXX {$28.00} and XX/XX/XXXX {$83.00} Lastly, as a military member if I am on XXXX XXXX per XXXX receiving XXXX care, I can not be billed during that time. I was on XXXX XXXX orders. I also had XXXX XXXX insurance. I changed to due to better options. The billing representative verbally stated to me that I had so many insurances. However she does pre authorization especially for my personal XXXX plans so I can know what my copay is. This is part of the scheduling process. Approval has to be made to ensure the insurance will cover it or the expense will be on the fault of the patient. I personally contacted the XXXX plans and they did see where billing came to there office but it passed the statute of limitations due to it originally arriving at the wrong location.

Consumer Complaint: This company which I have " NO CONTRACT '' with has no legal right to collect on this said debt. Reporting of the debt is a violation of the FCRA. Section 623 ( a ) ( 2 ), Section 623 ( a ) ( 1 ) ( B ), and Section 623 ( b ) ( 1 ) ( E ). Please provide me with a true and certified copy of the complete audit XXXXrail/Certified Final Forensic Internal Audit of said account. Furthermore, in order for this company to validate the debt, please provide verification through audit certification of debt entry in accordance with G.A.A.P., I.F.R.S., in accordance with basel 3 accord and U.N.C.I.T.R.A.L Conventions. Consumer Complaint: In XXXX of 2020, I found out that I had become a victim of identity theft. After being denied for a credit card, I saw a collection account on my credit report. The account had been opened with XXXX XXXX years prior and had never been paid on. The debt has been sold to several companies. I reached out to each debt collection agency and provided them with a letter notifying them that my identity had been stolen and that I would need the account to be removed. All responded stating that the debt was mine. I followed up by requesting that they provide me with any statements, payment slips, records of phone numbers that were used to activate the account, original contracts with my signature and a summary of their investigation to validate that the debt was mine. Alongside that, I requested that they notify the original creditor that the account was the result of identity theft and that the stop reporting the account to the credit agencies. None adhered to any of my requests and instead they all sent me a bill to pay for the collection.

Consumer Complaint: On XX/XX/XXXX I sent a request to verify the debt the company claim I owe. Few days ago I received account statements via mail. According to the Fair Credit Reporting Act, Section 609 ( a ) ( 1 ) ( A ), they are required by federal law to verify - through the physical verification of the original signed consumer contract - any and all accounts posted on a credit report. I ask to see Verifiable Proof ( an original Consumer Contract with my Signature on it ) they have on file. The failure to positively verify these accounts has hurt my ability to obtain credit. Under the FCRA, unverified accounts must be removed and if they are unable to provide me a copy of verifiable proof, they must remove the account.

Issue: Problem with a credit reporting company's investigation into an existing problem

Consumer Complaint: I sent information to XXXX, disputing an erroneous claim by a collection agency representing XXXX for an alleged unpaid bill. I sent to XXXX extensive documentation of my cancellation of the account, the reasons/proof behind the cancellation, and my extensive follow-up after I became aware of an issue. Its " investigation '' was apparently to confirm with the collection agency that the debt was valid. They have no proof or documentation that I did not cancel or agreed to continued insurance beyond the date for which I had paid. So, this was not a real investigation. Also, the collection agency has no judgment or other proof : they are just reporting false information ( submitted under another complaint. ) I requested a property insurance policy through XXXX in XX/XX/2019, as a requirement for a HELOC application. I signed up for auto payment. I enrolled in the plan through the phone and internet. All of my documents, signature pages and payments were electronic. In XX/XX/2019, a title issue with the property blocked the HELOC from moving forward ; I had been approved, but could not get funded as a result. I therefore notified XXXX in writing that I was cancelling the policy, since I had only added it to comply with the loan requirement. No auto payment was taken thereafter and I received no correspondence on the matter, so I had every reason to assume the policy had been cancelled as directed. In fall 2019, I became aware of a derogatory credit entry. It stemmed from XXXX 's claim that I owed 2 months premium for the policy which it did not cancel until XX/XX/2019. I contacted them immediately and described my cancellation. I was given several false answers and information, stating that XXXX did not accept written cancellation ( it's the law ), that it never received mine, that there were other reasons my auto payments had been stopped and also that all correspondence had been sent to the property address by mail, even though my account was completely e-notice AND it was an investment property occupied by tenants. XXXX also did not offer a landlord policy. When I provided all of my documentation, they stated that they would only " backdate '' my cancellation if I could show proof of another policy, which is completely irrelevant. The property remains uninsured and wasn't insured prior to the policy with XXXX, so that is a completely made-up requirement. The insurance company is XXXX and the collection agency is TransWorld System Inc.
