I am holding the accountable for violating the following code of the Fair Credit Reporting Act. 15 U.S.CODE 1681B PERMISSIBLE PURPOSE OF CONSUMER REPORT They did not get my written permission, nor did I give authorization this a violation of the Fair Credit Reporting Act P.L 90-321 ( 82 stat. 146 ) states that any agency can only get your report in response to a request by the head of the state or local child support enforcement agency ( or a state or local government official authorized by the head of such an agency 12CFR 1022.3 DEFINITIONS anything added to my/ your reporting without your consent can be considered identity theft. I wish to enforce 15 U.S.C 1681C block of information resulting from identity theft, I will provide the appropriate proof of my identity along with a copy of the XXXX REPORT. and also, The Privacy act of XXXX ( 5 U.S CODE 552a ) XXXX AND XXXX are a consumer reporting agency and I am a consumer, I have the right to make sure is not shared which is back by 15 USC 6801 which states " It is the policy of the Congress that each financial institution has an affirmative and continuing obligation to respect the privacy of its customers and to protect the security and confidentiality of those customer 's nonpublic personal information, " Furnisher of information to the credit agencies is a financial institution by definition under the title. 15 USC 1681 SEC 604 A SECTION 2 states that " In general subject to subsection ( C ), any consumer reporting agency may furnish a consumer report under the following circumstances and no other : in accordance with the written instructions of the consumer of which it relates. " FURNISHER OF INFORMATION TO CREDT AGENCIES ) the financial institution and the reporting agencies, XXXX and XXXX do not have my consent to furnish this information and they do not have written consent. XXXX and XXXX are not maintaining reasonable procedures. Also 12 CFR 1016.7 states that " A consumer may exercise the right to opt out at any time. '' I am opting out of your reporting services.

Consumer Complaint: On XX/XX/2022, I returned my XXXX XXXX equipment to the XXXX XXXX XXXX store to close my account. I notified the customer service agent that I would be moving and wanted to pay any remains balance that day. He gave me a receipt for my returned items and I left. I never received a balance or receipt of a balance. I also notified him of my new address, which is on my drivers license. After I closed my account, I spoke with a representative about the remaining balance that was required to pay. I was told that I would be able to pay the remaining balance on my XXXX app. My XXXX account was soon locked and I was unable to access the application. I made repeated attempts to contact XXXX by phone, but I was never able to pay the remaining balance because I was told that the account was now closed. For several months, I would attempt to contact XXXX XXXX and was given an automated message for hours. When I was able to reach a customer service agent, they could not access my full XXXX account number and told be I would receive emails or letters to correspond with the remaking balance. I moved to a new address and did not receive any mail attesting to the remaining balance from XXXX. I finally received a phone in XX/XX/XXXX from TSI, a collections agency, and agreed to pay a reduced amount of {$110.00} to settle to the debt collection of {$130.00} and remove the delinquent payment from my credit score. I paid the balance of {$110.00} by phone. I received no receipt following our correspondence via email or by mail to my new address. The representative notified me that XXXX would be notified in two business days of the resolution. On XX/XX/XXXX and XXXX, I received calls from another collections agency with the phone number ( XXXX ) XXXX regarding a debt from XXXX. When I notified them of my recent payment, I was told by the customer service agent on XX/XX/XXXX that the matter had not been resolved and I needed to provide him with my routing and account number to pay the remainder balance. I was very anxious and scared by his comments and began to doubt the validity of the call. I have been a XXXX XXXX customer for 10 years and have never experienced an ordeal like this. I have never missed a payment and feel some of these actions have been predatory in nature. I didnt not receive the proper correspondence or notification of my debt for the opportunity to resolve it. Further, my credit score has been adversely impacted and I fear my personal banking information is now widely available to several entities.

Consumer Complaint: On XX/XX/XXXX I rented a Compact car at XXXX XXXX Car through a reservation made at XXXX for {$360.00}, of which {$140.00} would be collected by XXXX at the pick up of the reservation. After concluding the rental I received a receipt where I was wrongfully charged an upgrade that with tax totaled {$220.00} in addition to the {$140.00} owed. I tried contacting XXXX to dispute this charge to no avail so I disputed it with my credit card through XXXX XXXX. XXXX Bank was able to successfully dispute the wrongful charge of {$220.00} but now I am being contacted by a Collection Agency, Transworld Systems Inc, saying that I still owe this money. I have spoke to many representatives at both XXXX XXXX and Transworld Systems Inc and have gotten nowhere. They constantly transfer me and put me on hold until I am disconnected. I do not know what to do about this wrongful debt as I thought it was resolved with my bank and do not want it to hurt my credit.

Consumer Complaint: Dear CFPB, I filed a complaint in XX/XX/XXXX and applied for Financial Assistance following the website on XX/XX/XXXX. Per the ACA enacted XX/XX/XXXX, XXXX XXXX which is a registered hospital system and 501 ( c ) 3 nonprofit per the IRS must provide and publish their FAP. I was a patient for multiple emergency care visits that lived well under 200 % FPL and uninsured for periods of time between XXXX and I was never once offered any FAP nor even provided any form of actual medical care during those time frames. I applied for FAP on XX/XX/XXXX once I realized there was a program and provided my Paystubs, Letter and Employment Verification from my employer, and explained all financial and mitigating circumstances that had happened and continue to happen in my life via email. They responded only in XX/XX/XXXX to state that I was above the income level. I provided screenshots of their financial aid policy from their website and they responded that the FAP on their website did not apply for XXXX but was only applicable for XXXX and that the FAP in XXXX was under 200 % of FPL. I requested verification of where on the website this was stated as FAP on website states 400 % and I am well below that income limit now and previously, provided multiple screenshots and PDFs from their website showing it was not listed anywhere that the current website documentation was only for XXXX hospitalizations, as well as requested a information for how to reapply for FAP, and also requested the written FAPs from XXXX at that time. XXXX XXXX stopped responding at the time. Website only states " We calculate the bill using the same amounts billed to people with insurance. '' but no information on what or where the XXXX is gathered. I confirmed with CMS Provider Fee Schedule and amount I was billed was incredibly higher than CMS. Lastly, I did a bit of research and found out that Delaware has 75 Del. Laws, c. 192, 10 ; 76 Del. Laws, c. 87, 3 ; 77 Del. Laws, c. 132, 2 which states that DE Hospitals seeking Certified Public Review status must follow Delaware Health Resources Board regulations which includes Charity Care and FAP offering. Delaware law authorizes the Delaware Health Resources Board to determine the charity care which states in XXXX " Eligibility and Charity Care Guidelines : Patients eligible for charity care are those individuals whose annual income is less than or equal to 350 percent of the Federal Poverty Level, as published annually in the Federal Register, and who are uninsured or underinsured ( i.e., overall medical expenses and/or health plan deductible equal to or exceeds 5 percent of annual income ). '' XXXX XXXX, who has received XXXX approval since XXXX per public websites, then is required by the state to follow the income requirement of at minimum 350 % FPL. The already was violated then as they told me that they would only consider XXXX income and that needed to be under 200 % FPL. Delaware Law states they must follow CMS billing rates for uninsured, indigent, and underinsured patients meeting financial requirements. I am attaching my paystubs for the previous 6 months, statement of employment verification signed my employer, and statement of need declaration that shows I live under 215 % FPL while DE states that charity care patients are uninsured and under 350 % FPL as well as follow the publicly accessible FAP. The medical bills have not been verified to you as an agency and no other organization is willing to help nor tell me where to go to even reach this government entity but the credit bureaus claim its from XXXX. I had medical insurance for some of that period too so but the hospital service did not even attempt to contact them for payment. I never received a bill or notification on financial assistance or even the bill to this date. I am hoping you can either direct me to the correct agency to file my complaint and have help removing this from my financial report as Account Closed due to insurance and Charity Care or if you could please help me directly to follow the current initiatives in XX/XX/XXXX to help those impoverished with medical debt. They address their letters to my late husband XXXX XXXX. The account number they use is XXXX. He passed on XX/XX/XXXX of XXXX XXXX XXXX XXXX XXXX XXXX XXXX XXXX XXXX XXXX XXXX, XXXX TX XXXX XXXX I contacted XXXX XXXX and spoke with XXXX in their financing dept. who confirmed that my husband has no debt with them, in fact has a credit of {$95.00}. My first letter to XXXX XXXX was sent by priority mail, my second one by certified, my third one was notarized. Each one addressed the fact that they are mistaken about the debt. XXXX XXXX has sent statements dated XX/XX/XXXX, XX/XX/XXXX, XX/XX/XXXX, XX/XX/XXXX, and then to a debt collection agency that temporarily suspended my case upon further inspection, XXXX XXXX has an answering machine but has never called me back after I left many messages at XXXX XXXX XXXX, It's as if this company no longer exists but by automatically set software to get peoples ' money. I suspect fraud and hope that CFPB can help me with this. Thank you.
