Despite my repeated efforts over the past two to three years, including specific requests in XXXXXXXX XXXX XXXX Lexis Nexis has failed to rectify inaccuracies in my consumer profile, in direct violation of the Fair Credit Reporting Act ( FCRA ) and the Fair and Accurate Credit Transactions Act ( FACTA ). In my request dated [ specific date in XXXXXXXX XXXX ], I asked for the removal of inaccurate phone records from my consumer profile. Despite Lexis Nexis, in a letter dated XX/XX/ ( case number XXXX ( XXXX ), consumer number XXXX ), acknowledging these records as unverifiable and stating their removal, the enclosed consumer file did not reflect these changes. This non-compliance is a direct violation of FCRA, which mandates the correction of any inaccuracies upon discovery ( 15 U.S.C. 1681i ).
