Consumer Complaint: XXXX XXXX XXXX XXXX. XXXX. # I.C. Systems XXXX # XXXX XXXX XX/XX/XXXX To Whom it May. I am in receipt of your notice dated XX/XX/XXXX. I have been in dispute with Since XXXX of XXXX, when I called and complained about the inability for XXXX to Live Up to the terms of their service agreement. XXXX has been unable to provide me the signal strength required to receive phone calls for my business, and the several attempts I've made to resolve this matter have met with failures on their part to live up to the terms of their service agreement. My home office is located on XXXX XXXX and the lack signal strength they provide in my town is widely recognized by dropped calls, continual impaired signal strength They acknowledged this and offered to send me a range extender that I was told would improve my service. This was in XXXX of XXXX, I agreed, and they never sent it. I called and told them and due to poor customer support, resulting in log wait times, outsourced foreign support representatives that had no ability to resolve my complaints. Each time I would explain the issues, it was like having to give a rudimentary class in English. Foreign representatives would need to transfer me to a the XXXX support, which was worse, and on the several occasions when I was able to talk to them, they failed to follow-up on anything that would cure this problem, or even returning my calls. It is my contention that XXXX could care less about fulfilling their contractual responsibilities. I had been left with no option but to transfer my phone service to XXXX mobile to resolve the problems. It is their failure to fulfill their contact obligations, and because of this breach of contract with XXXX I am disputing their charges in full, and further demanding payment for the phone lease in the amount of {$650.00} in return. Copies of my drivers license and social security card have been stolen. Multiple police reports have been made the perps have rerouted my mail back to old addresses I believe to obtain the mail. It's been a lot of mail i haven't received. I've found banks accounts with my business name and money transfered. Consumer Complaint: In late XXXX I discovered a collection on my credit report ( s ) from I.C. System, Inc. in the amount of XXXX XXXX, XXXX XXXX, XXXX Dollars and XXXX XXXX ( {$1100.00} ). I sent two separate debt validation request letters to I.C. System, Inc., both of which were sent via certified mail with return receipt requested. The first letter was sent on XX/XX/XXXX, and the second letter was sent as a follow-up on XX/XX/XXXX. Despite following the appropriate legal requirements for requesting debt validation, I have yet to receive any response from I.C. System, Inc. This lack of response raises serious concerns about the validity and accuracy of the debt in question. The FDCPA clearly stipulates that debt collectors must provide consumers with certain information upon requesting that I.C. System, Inc. prove that they are in compliance to collect such debt including the original contract with my signature of the debt, the name of the original creditor, and the amount owed. By failing to respond to my debt validation letters, I.C. System, Inc. is in violation of these provisions. I would like to emphasize that my intention is not to evade legitimate debts or disrupt lawful collection efforts. Instead, I seek transparency, accuracy, and fairness in the debt collection process. As a consumer, I have the right to verify the legitimacy of any debt being pursued.

Consumer Complaint: My son attended XXXX XXXX XXXX XXXX for several years as a XXXX XXXX. He had several services rendered under my insurance ( MEDICAID ), they referred him twice for XXXX and we were told it wouldn't be covered by the insurance that I have. After the second consultation which was because the prior one expired due to length in time since my decline, his father and the company setup payment arrangements. I believe the insurance used was XXXX or whatever system he signed up for, I do not know the details since I declined and did not have any part in the service agreement. I spoke with the both managers at the practice over the last two years on both sides regarding the issue. I advised them of the concern and was told that due to account setup they couldn't list both parties. In speaking with the manager of XXXX XXXX it was verified that medicaid was listed for his office XXXX exams and visits. In speaking with XXXX manager I continuously was given the run around and redirected to billing and asked for court documents which were provided several times. The facility sent the debt to collection in my name and still to this day I have yet to see a service agreement with terms, conditions or my signature. I spoke to the debt collector yesterday whom was not advised nor aware of the matter and stated that they would need to pull back the debt or we would need to go through court.

Consumer Complaint: According to the Fair Credit Reporting Act 15 USC 1681 section 602 a states " There is a need to insure that consumer reporting agencies exercise their grave responsibilities with fairness, impartiality, and a respect for the consumers right to privacy. '' XXXX and XXXX and XXXX are consumer reporting agencies and I am the Consumer. I have the right to make sure my private information isn't shared which is backed by 15 USC 6801 which states " It is the policy of the Congress that each financial institution has an affirmative and continuing obligation to respect the privacy of its customers and to protect the security and confidentiality of those customers nonpublic personal information. '' I.C. System is a financial institution by definition under that title. 15 USC 1681 section 604 a section 2 states that " In general Subject to subsection ( c ), any consumer reporting agency may furnish a consumer report under the following circumstances and no other : in accordance with the written instructions of the consumer to whom it relates. '' I.C. System the financial institution and the Consumer reporting agencies XXXX and XXXX and XXXX do not have my consent to furnish this information and they surely do not have my written consent. Any and all consent to XXXX, XXXX, XXXX, I.C. System whether it be verbal, non-verbal, written, implied or otherwise is revoked. 15 USC 6802 ( b ) ( c ) states that " A financial institution may not disclose nonpublic personal information to a nonaffiliated third party unless the consumer is given an explanation of how the consumer can exercise that nondisclosure option. I.C. System Never informed me of my right to exercise my nondisclosure option. Not only that 15 USC 1681C ( a ) ( 5 ) states " Except as authorized under subsection ( b ), no consumer reporting agency may make any consumer report containing any of the following items of information Any other adverse item of information, other than records of convictions of crimes which antedates the report by more than seven years. '' This account is an adverse item they are reporting again without my permission which is against the law. 15 U.S. Code 1681s2 ( A ) ( 1 ) A states " A person shall not furnish any information relating to a consumer to any consumer reporting agency if the person knows or has reasonable cause to believe that the information is inaccurate. 15 U.S. Code 1681e states '' Every consumer reporting agency shall maintain reasonable procedures designed to avoid violations of section 1681c of this title and to limit the furnishing of consumer reports to the purposes listed under section 1681b of this title. XXXX and XXXX and XXXX are not maintaining reasonable procedures. Also XXXX2 CFR 1016.7 states that " A consumer may exercise the right to opt out at any time. '' I am opting out of your reporting services.
