Consumer Complaint: In XXXX of 2020 I moved into my new home at XXXX XXXX XXXX XXXX in XXXX. My new home was new construction and when I called XXXX to transfer service from XXXX XXXX XXXX XXXX XXXX advised that they did not have the infrastructure to provide service to the newly constructed home. The tech on the phone explained that since XXXX was not available yet in my new neighborhood their would be no termination fee. I subsequently took the XXXX equipment to the XXXX store in XXXX and the store tech re-confirmed that there would be no termination fee. I returned the equipment and paid my bill in full and got a receipt. Subsequently, I received a bill from XXXX for the equipment and termination. I called them multiple times to remove the charges. Nonetheless, the bill was sent to a collection agency last year. I disputed the bill and remitted the receipt for the equipment. It has now been placed with a different collection agency ( IC Systems ) and they are offering a settlement. I called and explained this and they refused to provide an email address or fax number for written communication. The bill is for {$400.00} and I have already lost more than that amount in the loss of productivity because of the time spent on the phone. I would like this resolved immediately so I am not threatened with negative impacts to my credit by this fraudulent behaviour.

Consumer Complaint: On XX/XX/2021 I received a debt collection letter from I.C. System, Inc. On XX/XX/2021 I wrote to I. C. System, Inc. not to call or mail me any further collection debt stuff, that it will be considered harasment. I mailed the letter on XX/XX/2021. On XX/XX/2021 at XXXX XXXX from the number XXXX, the male representative stated that he was trying to collect a debt for I.C, System, Inc. I told the guy that I wrote a letter to his company on XX/XX/2021 to not contact me further via, phone or by US Mail, that it would be considered harassment. He stated that his company doesn't have a letter in my file regarding this info. XX/XX/2021 after the phone call I wrote a letter telling I. C. I do not recall giving permission toto I.C. SYSTEM, INC # XXXX, XXXX XXXX XXXX I # XXXX, XXXX & XXXX XXXX to release my medical information to a third-party. I am aware that HIPPA does allow for limited information about me but anything more than toto I.C. SYSTEM, INC # XXXX, XXXX XXXX XXXX I # XXXX, XXXX XXXX XXXX XXXX is to only be revealed with the patients 'authorization, therefore my request is two fold and as follows, Validation of Debt, and HIPPA authorization.

Consumer Complaint: Hi IC Systems ; Im confused and stressed and shocked when I reviewed my credit report this week and I found multiple inaccuracies on my credit report from the 3 major credit bureaus from a collection company that I never have dealt with before. I've attempted several times to ratify the issue with the original creditors and credit bureaus and they still reporting inaccurate, unverifiable information. I have disputed this item with the credit reporting agency and they reported you confirmed the account as valid. I honestly do not believe to ever have any relationship with this collection agency or original creditor. Also there are several inaccuracies with this account the way it's reporting and that's a total violation of FCRA and FDCPA laws and regulations. In a good faith effort to resolve the matter amicably, I must demand proof of this debt, specifically the alleged contract or other instrument bearing my signature, as well as proof of your authority in this matter. Absent such proof, you must correct any erroneous reports of this past debt as mine. I am writing to request that you please provide the following information : 1. Please evidence your authorization under 15 USC 1692 ( e ) and 15 USC 1692 ( f ) in this alleged matter. 2. What is your authorization of law for your collection of information? 3. What is your authorization of law for your collection of this alleged debt? 4. Please evidence your authorization to do business or operate in this state. 5. Please evidence proof of the alleged debt, including the alleged contract or other instrument bearing my signature. 6. Please provide a complete account history, including any charges added for collection activity. You have 30 days upon receipt of this letter to provide strict proof of contract. Consumer Complaint: Shortly after my XXXX were born in XX/XX/2018, I took them to XXXX XXXX for a pediatrician appointment. I had health insurance at the time, but when XXXX XXXX submitted the claim for payment, the claim was rejected as the insurer indicated I was not covered. I cleared up this issue with the insurer ( I was covered ) and the health insurer requested that XXXX XXXX resubmit the claim. XXXX XXXX refused to re-submit the claim. I resubmitted the claim myself but the insurer insisted that the provider resubmit it. XXXX XXXX again refused. The claim was apparently sold the receivable to IC Systems, who reported 2 claims for {$450.00} to XXXX ( there was only ever 1 claim ). I disputed these claims with XXXX and they were removed. Once removed from XXXX, IC Systems then reported 2 claims for {$450.00} ( again only ever 1 claim ) to XXXX. I disputed the claims with XXXX XXXX has apparently rejected the dispute. However, they did not issue a rejection notice - only a notice that they have updated the claim with new information. It appears they only updated the expiration date of the negative report, but it is unclear as there was no explanation provided
