I did not provide permission to be reported starting back when I ( beneficiary ) was XXXX old to this day nor has I ever given permission to ANY/NO CREDIT REPORTING AGENCY permission or instruction verbal nor in writing for the identified transaction to be reported on my ( BENEFICIARY ) CONSUMER REPORT. No one never told me ( BENEFICIARY ), I could opt-out because I have right to privacy. I am also requesting this information that is due to identity theft be block within 4 days being made on the basis of material misrepresentation of fact by me ( BENEFICIARY ) relevant to the request to block the identified transaction.
