Consumer Complaint: I scratched my XXXX. I saw a doctor at XXXX XXXX in XXXX ( XXXX FL ). I got a bill from the hospital and I paid it. Now an unknown entity is billing me without showing me a statement signed by me agreeing to have that entity provide services for me and that I agreed to pay that entity for their service. XXXX has no right to put this on my credit report without proof that I contracted with entity. I have proof that I paid the hospital. This is double billing and fraud.

They had no social security information on me as well as no Date of Birth information incorrect addresses. I spoke with a XXXX XXXX Hospital RepXXXX that verified with me that they do not collect Social Security Information or personal id information. The rep. also verified for me that there are 2 dates of service totally the amount of the debt. Also confirmed for me the person the debt belonged to was a person who lives XXXX, TN. This company has harrassed my parents home phone as well as my personal phone. XXXX an Item to a credit bureau on a person that did not verify there information.

I sent them a Affidavit of Truth stating to them that they had violated laws under 15 USC 1692. The violations committed were 15 USC 1692b ( 5 ), 15 USC 1692d ( 2 ), 15 USC 1692b ( 2 ), and 15 USC1692g. I have also attach proof of these violations.
