I received a copy of my credit reports and discovered this trade line on my credit report. I did not authorize this account or transaction. I did not apply for any credit with this company and have no existing accounts with XXXX XXXX. Please delete this information from my credit report, pursuant to section 605B of the Fair Credit Reporting Act. I have contacted both I.C. Consumer Complaint: I am a victim of identity theft. Due to the XXXX Virus Pandemic, we are all facing which has me sitting still at home and I saw the recent news about the multiple XXXX XXXX XXXX I decided to look at my credit reports from the XXXX major credit bureaus and found that someone had used my Identity. I have no idea how the theft took place. I also have no knowledge of any suspects. I did not receive any money, goods, or services as a result of identity theft. I contacted the Credit Bureau and told me to file an Identity Theft Report which I am doing. I appreciate your effort in getting this matter resolved. Thank you. Please let me know if you need any other information from me to block this information from my credit report. Thank you.

Consumer Complaint: Reviewed my credit report and a medical account was added I am allowed under the HIPPA law [ HEALTH INSURANCE PORTABILITY AND ACCOUNTABILITY ACT OF 1996 ], to protect my privacy and medical records from third parties. I am aware that HIPPA does allow for limited information about me but anything more than to I C SYSTEM COLLECTIONS # XXXX, XXXX XXXX XXXX # XXXX, XXXX # XXXX is to only be revealed with the patient 's authorization, therefore my request is two-fold and as follows, Validation of Debt, and HIPPAauthorization.
