Consumer Complaint: I am a victim of identity theft. Due to the Corona Virus Pandemic, we are all facing which has me sitting still at home and I saw the recent news about the multiple XXXX Data breaches. I decided to look at my credit reports from the 3 major credit bureaus and found that someone had used my Identity. I have no idea how the theft took place. I also have no knowledge of any suspects. I did not receive any money, goods, or services as a result of identity theft. I contacted the Credit Bureau and told me to file an Identity Theft Report which I am doing. I appreciate your effort in getting this matter resolved. Thank you. Please let me know if you need any other information from me to block this information from my credit report. Thank you.

Consumer Complaint: XX/XX/2019 : I receive a cat, and am concerned about the fact that she's drinking/XXXX excessively ( literally drinking a half-gallon per day ). I took her to XXXX in XXXX, and after a 5 min inspection and several attempts to pressure me into a wellness plan, they tell me she's fine with no testing at all. This was with XXXX XXXX. XX/XX/2019 : One of my two cats is XXXX XXXX, and we decide to take him into XXXX. We went to the XXXX XXXX location, where we saw XXXX XXXX. After running several tests ( and {$900.00} later ) they tell me there's nothing wrong with him. I then inform the Dr that my other cat has some issues with her XXXX and that XXXX XXXX said she was fine just a few months prior. A few days later : I was convinced to bring my cat back to the XXXX ( the one I adopted in XXXX ) so they could run additional tests to prove whether or not she was actually sick ( since they ran zero tests before telling me she was fine in XXXX ) and they informed me nothing was wrong yet again. During this visit, they convinced me to sign up for a wellness plan with a spay, because I was informed she would surely get XXXX XXXX if I didn't. This was now the second chance that the Dr 's have had to examine her. A few days later again : I get a call from the original location in XXXX ( with no communication from the XXXX XXXX office about her tests results ) that she needs to come in immediately for a comprehensive exam. at this time, I called the XXXX XXXX office asking for the test results, and an employee promised a call back by the end of the day. XXXX rolls around and I call again, only to be told it was the employees last day and they must have forgot to tell someone, so sorry we will call you tomorrow ( they did, still no real results ). During the visit to the XXXX XXXX ( the 3rd time they've had a chance to examine her ) they put me on the schedule for a spay the next week. Today : I went in for the scheduled XXXX, and a few hours later I get a call that the Dr. found a tattoo and scar on my cat AFTER PUTTING HER UNDER XXXX AND SHAVING HER that indicates that she was ALREADY XXXX!!!! Stay tuned, I'm still fighting with them to let me out of the wellness plans for my two pets ( not even a month later ) and I'm now being told that there were an additional {$600.00} in ADDITIONAL charges that were " billed to the plan '' that I would have to pay to cancel!!! If you're still not convinced, please see the countless reviews from former employees online here 's a quote from one " Pay is based on production ; as an example, you receive {$0.00} production for exams on patients on wellness plans! Minimum {$2400.00} billed services/ day. if not, you guessed it- not meeting production! Further, you are forced to push all sorts of ad ons, many increase hospital revenue but add nothing to your daily production. Sense a trend? '' At this point, I've spent well over {$1000.00} with no answers as to what is wrong with either of my cats, and I've wasted a crazy amount of time with these clueless XXXX 's. Save yourself the trouble and just go somewhere else. I called their corporate office about this issue and was not only talked over and laughed at but they literally haven't even acknowledged their wrong doing or apologized. I have attempted to call them SEVERAL times, including today with no callback.

I am XXXX years old and did not realize a relative had used my Personal information. That relative is in the XXXX XXXX system for the next 10 years. This money was being taken from my social security check and was discovered when the relative was XXXX. I have written to XXXX including a certified letter with no response. I always pay my debts but this is not mine.

Consumer Complaint: I have become aware of inaccurate information on my credit report because I wanted to refinance my home mortgage. With the information received on the credit report, I am not able to take advantage of the offers that would have been available had my credit report been accurate. I am submitting a complaint because there is an account recently added to my credit report identified as Collection when, in fact, the bill was paid upon receipt in XX/XX/XXXX. I visited the XXXX XXXX XXXX ER on XX/XX/XXXX following an automobile crash. The hospital took the auto insurance to process billing. Attached is documentation of the bill and payment of the bill from XXXX XXXX XXXX with service location noted as XXXX XXXX XXXX Aside from the gross inaccuracy of the information that I need rectified, I am disputing the timing of the report and how it is presented on my credit report. The reporting agency, IC Systems, is reporting an account opened XX/XX/XXXX and closed XX/XX/XXXX and reported XXXX, with manner of Payment " Collection ( I9B ) ''. Payment History indicates a Balance of {$500.00} listed in XXXX, XXXX, XXXX, XXXX, and XXXX of XXXX. Since learning of this via my mortgage broker, I have received a paper bill ( " Collection Notice '' ) in the mail, dated XX/XX/XXXX ( also attached ). Please let me know when this will be removed from my credit report. If not removed in a timely manner, I will pursue legal action.

Consumer Complaint: On XX/XX/XXXX I mailed a dispute to IC Systems and to the credit bureaus regarding a fraudulent account on my credit report. ON XX/XX/XXXX I noticed they verified the information and proceeded to call IC Systems to get to the bottom of this situation. I was informed, based on a search of my social security number, that there was an account sent to collections from XXXX XXXX ( AKA XXXX ) for a service address of XXXX XXXX XXXX XXXX in XXXX, CA that was opened in XX/XX/XXXX. I advised them this was not me, as I purchased my home in XXXX in XXXX and had no reason to open up any service in a different city. They advised me to contact XXXX and to " resolve it with them '' meanwhile they will remain on my credit report until XXXX advises them to remove it. This is completely abusive and unacceptable. ESPECIALLY because XXXX can not locate any account using my name, social, date of birth or address. This limits my ability to start a fraud investigation with XXXX as advised my IC Systems since they can not locate an account to initiate an investigation on. What am I supposed to do other than go round and round in circles wasting my time?? This item needs to be removed from my credit report ASAPPPPPP

Upon further research, this account was transferred and I am no longer reliable for this account. Debtor 's creditor did not verify this collection. Also, there was no written form of communication, contract or signature for this accused debt at all. A bill without a signature does not validate a debt and anybody can send me a bill and say it's mine. According to the FCRA, I C SYSTEM has violated my rights as a consumer and I'm requesting legal action to remove this account immediately.

When I tried talking to them, they were not very helpful and I still can not remember this debt.
