My name is XXXX XXXX XXXX, and I am concerned about a reported late or missed payment. Pursuant to my rights under 15 USC 1681E ( b ), I request a prompt investigation into this matter. I also kindly urge you to rectify the reported late or missed payment on my account. It is crucial for my credit history and financial standing that accurate information is reflected in my records, as this incorrect information is currently defaming my character. This matter has recently caused me mental anguish, as well as embarrassment because I was denied credit on multiple occasions because of this matter at hand. Furthermore, 15 USC 1681a ( d ) ( 2 ) ( B ) provides for all extensions of credit which were derived from a credit card to be excluded from consumer reports. This means you broke federal law knowingly and willfully, which makes you liable under 15 USC 1681n, which carries penalties of {$1000.00} per violation.
